Sandy Pogones
Sandra Pogones has over 25 years' experience in healthcare quality improvement and project management with significant hands-on work with physicians. As Program Manager for Physician Services with Primaris, Pogones applies principles of improvement science to assist physicians in documenting, monitoring, reporting and improving preventive and chronic care using the EHR as a tool. Sandy has assisted physicians with implementing EHRs, streamlining workflows, integrating evidence-based quality measures into practice operations, and earning incentive payments/avoiding penalties under Meaningful Use, PQRS, Patient-Centered Home programs and value-based purchasing. Pogones has participated with the Office of the National Coordinator's Regional Extension Center services to identify barriers to effective EHR use and to strategize solutions. She is an Institute for Healthcare Improvement (IHI) “Breakthrough Series” graduate, an active participant in IHI's clinical office practice redesign and improvement initiatives, and has participated in Mayo Clinic’s High Performance Physicians Institute.
